How bone actually works — a living, remodelling tissue

Bone is not inert scaffolding; it is continuously broken down and rebuilt by two cell types working in balance — osteoclasts that resorb old bone and osteoblasts that lay down new. Underneath the mineral is a protein scaffold, mostly type I collagen, onto which calcium phosphate crystals are deposited. That is why bone is both hard and slightly flexible: the collagen gives tensile strength, the mineral gives rigidity. Support strategies that address only calcium miss the protein framework and the signalling that decides where calcium ends up.

What to look for — the cofactors that direct calcium

Calcium needs a supporting cast. Vitamin D is required to absorb calcium from the gut in the first place. Vitamin K2 then activates the proteins (osteocalcin and matrix Gla protein) that escort calcium into bone and help keep it out of arteries — which is why D and K2 are increasingly paired. Magnesium is needed to convert vitamin D into its active form and is itself a structural component of bone. Boron, silica and adequate protein round out the picture. When choosing calcium, the form matters: citrate is well absorbed and does not depend on strong stomach acid, while carbonate needs to be taken with food.
