How it works — a universal antioxidant and metabolic cofactor

Most antioxidants are confined to one compartment of the body — vitamin C to water-based environments, vitamin E to fatty ones. Alpha-lipoic acid is distinctive in being both water- and fat-soluble, so it is active throughout the cell, which is why it's often called a "universal" antioxidant; it can also regenerate other antioxidants (vitamin C, vitamin E, glutathione and CoQ10), extending their working life. Beyond this, it is an essential cofactor for the mitochondrial enzyme complexes (including pyruvate dehydrogenase) that convert glucose into energy — placing it at the centre of glucose oxidation, which underlies its study in glucose metabolism and nerve health. Thiamine (B1) is a cofactor for the same complexes; riboflavin (B2) supports the regeneration of lipoic acid.
